What is Proxy Access?

Proxy access enables a nominated individual—such as a family member, carer, or trusted friend—to access and manage specific elements of your healthcare services. This can include:

• Booking and viewing appointments
• Ordering repeat prescriptions
• Accessing parts of your medical record, such as test results or vaccination history

This service is particularly beneficial for:

• Patients with physical or mental health conditions that make managing healthcare challenging
• Elderly patients who may need assistance
• Parents or guardians managing healthcare for children

How to Set Up Proxy Access

To establish proxy access:

1. Complete the Consent Form Below
   Both you and your nominated proxy will need to fill out the online consent form provided on this page.
2. Provide Identification
   Both parties must provide valid photo identification, which needs to be submitted in person.
3. Approval Process
   The practice will review the application to ensure it meets all criteria and is in the patient’s best interest.

Please note that granting proxy access is subject to approval, and the level of access may be limited based on individual circumstances.

Important Considerations

• Consent
  Patients aged 16 and over must provide explicit consent for proxy access. For those unable to consent due to incapacity, decisions will be made in their best interest, following NHS guidelines.
• Confidentiality
  The practice is committed to maintaining patient confidentiality. Proxy access will be granted with careful consideration to protect patient information.
• Revoking Access
  Patients have the right to revoke proxy access at any time by notifying the practice in writing.

The NHS App makes it easier than ever for proxies to assist patients with managing their healthcare. With proxy access enabled, a nominated individual can securely:

• Book and manage appointments
• Order repeat prescriptions
• Access medical records, including test results and immunisation history

How to Set Up Proxy Access on the NHS App

To enable proxy access on the NHS App:

1. Complete the Consent Form Below
   Fill out the online proxy access form provided on this page.
2. Provide Identification
   Submit valid photo identification for both parties as instructed.
3. Approval Process
   Once approved, the practice will link the proxy’s NHS App account to the patient’s medical record.
4. Confirmation
   The proxy will receive a notification confirming access and can begin managing healthcare through their NHS App account.
