How to Set Up Proxy Access

To establish proxy access:

    Both you and your nominated proxy will need to fill out the online consent form provided on this page.
  2. Provide Identification
    Both parties must provide valid photo identification, which needs to be submitted in person.
  3. Approval Process
    The practice will review the application to ensure it meets all criteria and is in the patient’s best interest.

Please note that granting proxy access is subject to approval, and the level of access may be limited based on individual circumstances.

  • Consent
    Patients aged 16 and over must provide explicit consent for proxy access. For those unable to consent due to incapacity, decisions will be made in their best interest, following NHS guidelines.

How to Set Up Proxy Access on the NHS App

To enable proxy access on the NHS App:

  2. Provide Identification
    Submit valid photo identification for both parties as instructed.
  3. Approval Process
    Once approved, the practice will link the proxy’s NHS App account to the patient’s medical record.
  4. Confirmation
    The proxy will receive a notification confirming access and can begin managing healthcare through their NHS App account.
