If you suspect you have Coronavirus DO NOT visit the Surgery as advised call NHS 111

Travellers from Wuhan and Hubei Province

If you have travelled from Wuhan or Hubei Province to the UK in the last 14 days you should immediately:

  • stay indoors and avoid contact with other people as you would with the flu
  • call NHS 111 to inform them of your recent travel to the area

In Scotland phone your GP or NHS 24 on 111 out of hours. If you are in Northern Ireland, call 0300 200 7885.

Please follow this advice even if you do not have symptoms of the virus.

Travellers from other parts of China and other specified areas

This advice applies to travellers who have returned to the UK from the following areas:

  • China
  • Thailand
  • Japan
  • Republic of Korea
  • Hong Kong
  • Taiwan
  • Singapore
  • Malaysia
  • Macau

If you have returned to the UK from any of these areas in the last 14 days and develop symptoms of cough or fever or shortness of breath, you should immediately:

  • stay indoors and avoid contact with other people as you would with the flu
  • call NHS 111 to inform them of your recent travel to the country

In Scotland phone your GP or NHS 24 on 111 out of hours. If you are in Northern Ireland, call 0300 200 7885.

Please follow this advice even if your symptoms are minor.

keyboardstethIf you wish to register as a patient at this practice, and your address falls within our catchment area you will need to download and complete the following forms:-

Health Authority GMS1 New Patient Registration Form (PDF file)

ADULT New Patient Questionnaire

UNDER 12 New Patient Questionnaire

Information for visitors from abroad

Once completed you will need to bring them with you to the surgery along with the documents below (these MUST be dated within the last 3 months). Inability to provide any of the necessary documentation please speak to Reception about your Registration.

  • You need to provide your NHS NO at the time of registration (you will be able to obtain this by asking your previous surgery or any Hospital letters you may have will have your NHS NO on)
  • One form of Photographic ID (e.g. Passport; UK Drivers Licence; Student Card)
  • One proof of address (e.g. Gas, Water, Electricity, Phone or Council Tax Bill; Bank Statement; or a letter from your landlord or housing department to confirm your residency.  This must be dated in the last 3 months)

After submitting all of the above documentation to either of our Surgeries, please allow 48 hours before making an appointment to allow for the completion of the registration process.
